RN Resource Nurse Care Transitions - UR/CM/Discharge Planner - FT
- FT (2 positions), 80 hrs/pp, 1.0 FTE; days, Mon.-Fri., varies as scheduled.
Provides Utilization Review, Case Management, Discharge Planning, and Swing Bed admission Co-ordination. Responsible for ensuring completion and updating of required assessments and notifications
according to timetables set forth by state and federal regulations, as well as payor contracts Assess for admission and continued stay criteria and assist in issuing letters of non-coverage for Medicare patients as
required by CMS standards. Attends weekly Interdisciplinary Team Meetings for Swing Bed patients. Participates in marketing of Swing Bed Services. Complies with insurance standards and requirements for
continued coverage of stay. Prepares reports for members of treatment team, reflecting patient’s reactions and evidence of progress or regression. Monitors/audits Nursing and Therapy documentation to ensure
compliance with state and federal regulations. Responsible for coordinating utilization review tasks including review of medical records for appropriateness, level of care and resource utilization. Assists with
denial process. Provides ongoing education to staff and providers. Able to function as the House Supervisor and provide nursing care in two nursing departments.
Education and/or Experience Requirements:
Graduate of accredited school of nursing.
WA State RN licensure.
A minimum of two years of experience as an RN, one of which will be in direct patient care of elderly and/or chronically ill patients. Knowledge of long-term care environment or rehabilitation.
Licensure & Certifications Requirements
Valid/unrestricted license as a Registered Nurse in State of Washington.
Must have BLS upon time of hire or within 30 days of employment.. ACLS, PALS, & NRP within 6 months of hire.
