Job Description

FT, 80 hrs/pp, 1.0 FTE; days, Mon.-Fri., schedule may vary.

Responsible for coordinating Social Services to the patients of the hospital and outpatient services. Specific duties include crisis

intervention, emotional support, establishing linkages with community resources and discharge planning. Provides case management

for the Emergency Department Information Exchange (EDIE) consistent care program. Provides support to the Acute Care Manager in

maintaining the operations, policies and procedures for the Social Service Program.  Required:  Master’s or Bachelor’s Degree in

Social Work required. Registered to practice in WA State as an Agency Affiliated Counselor or achieve such certification within sixty

(60) days of employment per RCW 18.19.210/WAC 246-810. Three years in a social service or health care setting required including

experience with counseling and discharge planning. Ability to recognize age specific needs and behaviors. Ability to communicate

both verbally and through written reports with strong documentation skills. Good interpersonal and time management skills.

Preferred:  Skilled nursing facility experience.
