FT 80 hrs/pp, 1.0 FTE; varies as scheduled.


The RHC Care Gap Coordinator is responsible for identifying, managing, and closing preventive care and chronic disease care gaps across RHC patients. The role coordinates outreach, workflows, and documentation to improve quality measures, care continuity, patient experience, and performance on value based programs. The coordinator works across clinical teams, Population Health RNs, providers, and administrative staff to ensure timely follow up, scheduling, and accurate charting.

Required: Knowledge of rural healthcare clinics with at least two years of experience working in the outpatient clinic setting. Effective knowledge of computer systems and software, including word processing, spreadsheet, database, clinical information systems, electronic medical records, EPIC experience preferred, billing systems, and other application packages to perform all required responsibilities. Effective written and oral communication skills. The ability to interact with staff, providers, and patients successfully and positively is critical.


Compensation & Benefits

  • Pay Range:  $20.87-$30.19
  • Incentive Compensation: Eligible for up to 5% of annual salary through the incentive program
