  • PT with benefits, 48 hrs/pp, .6 FTE; varies as scheduled.

Job Summary:

This position will coordinate efforts with others to provide post discharge follow up care coordination for patients recently discharged from inpatient or outpatient services. This position will also work collaboratively with clinical care teams in the coordination of care for certain patient symptoms, conditions and follow up care needed.

Required: 

WA State RN licensure. Minimum three years’ experience as a registered nurse in an in-patient or outpatient clinic setting. Demonstrated clinical skills and clinical expertise with an emphasis on patient triage. Demonstrated skills for problem solving & decision making. Understands patient safety requirements, quality improvement models, and the transitional interfaces between clinic, hospital, and community. Washington State Healthcare Professional Telemedicine Training within 30 days of hire. Telephone Triage as Professional Nursing Practice online course within 90 days of hire.

 Preferred: Bachelor’s degree. Telephone triage experience highly preferred. Spanish speaking preferred. Must otherwise be able to use telephone interpreting service. 

 Wage: $36.71 - $66.45
