RN Outpatient Care Transitions Resource Nurse - FT

FT, 80 hrs/pp, 1.0 FTE; varies as scheduled.
Job Summary:
Provides Utilization Review, Case Management, Discharge Planning, and outpatient care coordination. Prepares reports for members of treatment team, reflecting patient’s reactions and evidence of progress or regression. Responsible for coordinating utilization review tasks including review of medical records for appropriateness, level of care and resource utilization. Provides case management for the Emergency Department Information Exchange (EDIE) consistent care program. Provides ongoing education to staff and providers.
Required: WA State RN licensure. Graduation from an accredited School of nursing. A minimum of two years of experience as an RN, one of which will be in direct patient care of elderly and/or chronically ill patients. Knowledge of long-term care environment or rehabilitation. Must be computer literate, experience in Microsoft Office Suite and Electronic Medical Record. Demonstrated clinical skills and clinical expertise with an emphasis on patient triage. Demonstrated skills for problem solving & decision making. Ability to communicate both verbally and through written reports with strong documentation skills. Understands patient safety requirements, quality improvement models, and the transitional interfaces between clinic, hospital, and community.
Preferred: Telephone triage experience highly preferred. Certification in Case Management.
