FT, 80 hrs/pp, 1.0 FTE; days, varies as scheduled.
Job Summary:
The individual in the Population Health Registered Nurse (RN) position reports to the Outpatient Special Procedures Nursing Manager. Supports population health initiatives focusing on wellness, prevention and chronic care management. Collaborates, communicates and coordinates with the Provider and other health care team members in planning and implementing patient care. Understands and has knowledge of Medicare documentation, billing and reporting requirements associated with Annual Wellness Visits (AWV), Chronic Care Management (CCM) and Transitional Care Management (TCM) services to include patient calls and calculation of billable time. Performs face to face visits such as Medicare Annual Wellness Visits. Conducts injection clinics as appropriate (e.g. allergy injections, INR). Performs testing as directed by a provider (e.g. EKG). Helps providers close patient care gaps (e.g. colonoscopies, mammography, A1c, etc.). Preferred: Previous experience in patient care setting and surgical clinic experience.
Licensure & Certifications Requirements
WA State RN licensure.
BLS required, current AHA Healthcare Provider training (BCLS); ACLS & PALS desired within 6 months of hire
Wage: $36.71-$66.45
